Children's Recovery Center has a full time Social Worker/Case Manager whose sole responsibility is to support the families during their stay and to assist with Discharge Planning. This individual is committed to assisting the family with the challenges of caring for a medically complex child. Additionally, they routinely meet with families to provide assistance, resources and information that will help families support their child through the transition process.
The Social Worker/Case Manager is also committed to working with each family to assist them in the discharge planning process. The CM/SW assists the family by arranging nursing, durable medical equipment and any supplies the family will need at home. Additionally the CM/SW will coordinate with all outside agencies to provide services to the child at home including school, regional center, physician flu, therapy and nursing.
